Documents Required for Nurse Aide Course Registration:

  • Proof student is 17 years old at time of registration is required for Nurse Aide I.
  •  High School or GED Transcripts.
  •  Higher Education Transcripts if applicable.
  • Driver’s License, College or Picture ID.
  •  Social Security Card.
  • If the student is under 18 years old, a notarized parental consent form must be submitted.

Nurse Aide I Course Requirements and Fees:

  • Background Check /12-Panel Drug Screen – $80 (Information will be given out at registration).
  • TB Test (Two-step)– $30 Health Department
  • Physical Proof of Vaccination History including:(Price will vary by vendor)
  • 2 Measles/Mumps/Rubella vaccinations or positive antibody titer
  • History of Chicken Pox or 2 Varicella immunizations
  • Tetanus vaccination within the last 10 years
  • Flu vaccine (Seasonal)
  • Have had either the Hepatitis B vaccination series, be in process or have signed a waiver form.
  • CPR Certification – $4 if taken as part of the course – Outside of NA course price Varies By Instructor/vendor.
  • Ceil Blue Scrubs –  (Price will vary by vendor)
  • White Sneakers or Nursing Shoes– (Price will vary by vendor)
  • Textbook and Workbook –$75.00 (CFCC Book Store – Prices are for new books only and are subject to change). Textbook: Hartman’s Nursing Assistant Care: Long-Term Care (4th Edition) Susan Alvare Hedman, Jetta Fuzy, RN, MS and Suzanne Rymer, MSTE, RN-BC, LSW.

**The CFCC Nurse Aide I Program now offers In-facility testing for the North Carolina State Nurse Aide I Written Examination and Skill Evaluation. The NC State Testing Fee – $120.00.  The NC State Test is administered by Pearson Vue at CFC

  • Nurse Aide II Registration Fee:  $200.00 (Money Order Only)
  • Proof student is 18 years old at time of registration is required for Nurse Aide II.
  • Completion Certificate from a NC State Department of Health Approved Nurse Aide I Course.
  • Proof of being on the NC State Nurse Aide I Registry.
  • High School or GED Transcripts.
  • Higher Education Transcripts if applicable.
  • 18 yr old High School students not in a CCP/CTE Pathway program, must submit a recommendation letter (on school letterhead) from their high school principal or his/her designee.
  • Driver’s License, College or Picture ID.
  • Social Security Card.

Nurse Aide II Course Requirements and Fees:

  • Registration Fee– $200.00(Money Order Only)

Nurse Aide Level II Refresher/Competency Assessments

*Competency Assessment is only available for those NA II who have an expired NA II listing and meet all eligibility requirements listed below.

The NA II is eligible for Competency assessment in one of two ways:

  1. The NA II must have worked at least eight hours for compensation during the past 24 months performing nursing care activities under the supervision of a Registered Nurse.
  2. The NA II has had a continuous period of 24 months during which nursing care activities were not performed for monetary compensation, but Patient care activities were performed for Compensation

**The NA II must have no substantiated findings of abuse, neglect, or misappropriation of funds on the DHHSR Nurse Aide Registry.

NOTE: If a NA II does not meet either of the above criteria, an entire NA II course must be completed.
