Below are all the forms necessary to create, participate in, and operate a club\/organization at CFCC.<\/p>\n New Club Application <\/a> \u2013 Interested in starting a new club? Click on the following link for more information and instructions: New Club Information. Read the instructions carefully. Then complete the \u201cNew Club Application\u201d online form and submit.<\/p>\n Club Renewal Form <\/a> \u2013 Each academic year clubs are required to submit a \u201cClub Renewal Form\u201d in order to maintain their active status. Please click on the link and complete the online form.<\/p>\n Advisor Agreement Form <\/a> \u2013 Any CFCC employee who agrees to serve as a club advisor must follow the expectations and responsibilities set forth in this document. Please sign and submit the form to Donterio Perkins in person in U-152, by interoffice mail, or scan by email to drperkins423@cfcc.edu <\/a> .<\/p>\n Activities Approval Form (Activity\/Event\/Fund Raisers) <\/a> \u2013 Clubs interested in organizing an on-campus activity, event, or any fundraising event should fill out the online \u201cActivities Approval Form (Activity\/Event\/Fund Raisers)\u201d at least three weeks prior to the date of the event for approval.<\/p>\n Off-Campus Field Trip Approval Form <\/a>

 Funding Request <\/a> \u2013 Funding for leadership, cultural, educational, recreational, social, service and civic activities sponsored by active CFCC clubs\/organizations or academic programs\/departments. Click on the following link for more information and qualifications: Rules for requesting funds. <\/a>

 Community Service Form <\/a> \u2013 Clubs interested in engaging in any type of community service project must fill out the \u201cCommunity Service Form\u201d three weeks prior to the scheduled date of the project.<\/p>\n Activity Waiver Form \u2013 Individual <\/a> \u2013 Any individual participating in an off-campus activity is required to complete, sign, and submit the \u201cActivity Waiver Form \u2013 Individual\u201d form before the activity. Please submit the form to the Student Life Coordinator.<\/p>\n Activity Waiver Form \u2013 Group <\/a> \u2013 Any group, club participating in an off-campus activity is required to complete, sign, and submit the \u201cActivity Waiver Form \u2013 Group\u201d form prior to the activity. Please submit the form to the Student Life Coordinator.<\/p>\n SGA Executive Officer Application <\/a> \u2013 Interested students can contact Donterio Perkins, SGA Advisor at drperkins423@cfcc.edu <\/a> .<\/p>\n Senator Application <\/a> \u2013 All students are members of the SGA.
