CFCC Employee Incident Reporting Form

This Incident Reporting Form is not for Emergency Services. Please call 911 if you have an emergency presenting an immediate threat to life, health, property or environment. You may use this form to report the incident to Human Resources.

Please provide a detailed description of the incident/concern using specific concise language. Please provide as much detail as possible in describing the behaviors and activities that occurred. The use of direct quotes, including profanity or abusive language, is encouraged. Avoid using pronouns when multiple individuals are involved. Please use full names and include as much description as possible regarding the circumstances that resulted in the report. Please include any photos, emails, texts, Snapchat, and/or supporting documentation.
