Documentation Requirements

Disability Support Services (DSS) is committed to providing support to students with disabilities. Students who have a disability are encouraged to arrange a confidential discussion by contacting the DSS office, by calling 910-362-7800 or emailing

In order to qualify for services, students must self-disclose and provide up-to-date documentation of their disability. The documentation must include a diagnostic statement, diagnostic tools/methodology used to evaluate the condition, and current treatment. The information must also document the functional impact of the disability, include recommendations for accommodations and state the credentials of the diagnosing professional.

In order to document a Learning Disability, a full diagnostic report that includes all standard test scores, subtest scores, and the evaluator’s narrative is necessary. The evaluation tool should be designed for adults rather than for children.

An Individual Educational Plan (IEP) / 504 Plan from a public school may be helpful in order to establish a history of accommodations, however, it may not be adequate on its own to document a disability.

The DSS Office has developed several Disability Verification Forms in order to help document certain disabilities.  Refer to the information below in order to access a specific form in the preferred format:

Once adequate documentation has been submitted to DSS, the student will then be required to meet with a DSS staff member to complete the registration process.  Students should  contact a DSS staff member  in order to schedule the required Intake Meeting.
