Documentation Requirements
To qualify for services, students must provide up-to-date documentation of their disability. The documentation must include a diagnostic statement, diagnostic tools/methodology used to evaluate the condition, and current treatment. The information must also document the functional impact of the disability, include recommendations for accommodations and state the credentials of the diagnosing professional.
To document a Learning Disability, a full diagnostic report that includes all standard test scores, subtest scores, and the evaluator’s narrative is necessary.
Disability Documentation Examples
- Psychological Evaluation
- Comprehensive Clinical Assessment (CCA)
- VA Disability Rating Letter
- Vocational Assessment
- Have your treatment provider complete the SAS Disability Verification Form
OR
Documentation can be submitted in-person at the SAS office (Union Station – 218), via fax: 910-362-7113 or via email: sas@cfcc.edu
Note: An Individual Educational Plan (IEP) / 504 Plan from a public school may help establish a history of accommodations. However, it may not be adequate on its own to document a disability.
